If he still is in a student-profession relation to you – does he grade your work or judge it in any way? – the prof is very much in violation of university norms. Never mind how it would reflect on you – it could get him fired, or at least in a lot of trouble. Plus you have to assume he's done this before with other students.
If you feel he had sex with you so impaired you couldn't reasonably be expected to consent, that's a crime. Again, he may be a serial offender.
If you're avoiding your math class, that's an example of why these sorts of relationships are forbidden. Even after the “incident” is over, he's continuing to negatively impact you.
I think you should report him. Ask for an interview with your dean of students, explain your misgivings. They will know what to do.
