Yeah you need to call him up before this date and be honest. If you don't, you are leading him on and an a-hole and it will come back to bite you in the end.
It's also possible he has only been your friend because he has always been interested in you. If you reject him, his attitude towards you could change, but that is because he is no longer putting in the extreme effort it takes to be your friend. So don't feel bad if he is no longer your best friend anymore, as it might have been an illusion he created to get on your best side.
John was basically a stand in as he’s the husband of one of my wife’s bridesmaids and she wanted it even on both sides. We had a 100+ person wedding but the bridal party itself was very small.
