Something's not adding up here. The title says that he broke up with you on your birthday, yet your post mentions you told him to leave and stay with his mom. Sounds like you are the one who broke up with him?
Other than that, it sounds like you have both communicated your desires. You have asked him to pay more attention to you and be less emotionally distant. He so far hasn't done so.
He has expressed that he wants you to lose weight and work on your self-esteem. It sounds like this is the first time you are hearing of it from him, so I don't expect you'll make that happen overnight.
Both of you, understandably, want to be loved as-is.
The only options I can see here are that you get couples therapy or divorce. I am not a big fan of the “just dump him/her” mentality of Reddit because I think marriage and kids are important. However it sounds like you both have work to do to make this work. I think you owe it to your kids to try couples therapy and/or individual therapy. If he does not agree to that for your sake or the kids sake, then I don't think there's much else you can do but move on.
